FINGERPRINTING PROCEDURES

All education students are required to have a criminal background check that involves fingerprinting prior to student teaching (in August for Fall semester student teachers and December for Spring semester student teachers).

Before you get fingerprinted, print off and complete the LiveScan Agency Fingerprint Data Entry Form at the link below (Form 11). You must bring this form with you to one of the locations listed below.

Fingerprinting can be done locally at the locations listed below:

Holland Nurses Stat of Michigan, Inc., 100 Waverly, Holland, MI 49423 (next to Wyandotte Federal Credit Union). Open ONLY on Monday, 9:00a.m.-11:15a.m. & 12:35-4:00 p.m.). By APPOINTMENT ONLY via IDT at (866)226-2952, (810)962-5449, (810)874-3669 OR on-line at www.idtfingerprint.com

You must bring a valid photo ID, and the Live Scan form located above.

Ottawa County Sheriff Department at the Fillmore Complex located at 12220 Fillmore Street, West Olive (between Waverly/120th and US 31). Phone number: 738-4000. WALK IN between 8:00-4:00 Monday-Friday: NO APPOINTMENT NEEDED (CASH ONLY)

You must bring proof of your local Ottawa County address (e.g., copy of address off of KnowHope or mail sent to campus), a valid photo ID, and the LiveScan form located above.

Your local Sheriff's Department where you reside.

You must bring a valid photo ID and the LiveScan form located above.


FINGERPRINTING RESULTS

Hope College is now working with the Professional Educational Services Group (PESG) for housing results of criminal background checks and processing paperwork for substitute teaching. PESG works with all school districts in Ottawa Country with the exception of Grand Haven Public Schools and most other counties in the area (Kent, VanBuren, Allegan).

The results of your background check will be sent directly to PESG if you utilize their LiveScan Agency Fingerprint Data Entry Form above. At the beginning of each semester, the Education Department will submit a list of student teachers to PESG to request the names of any students for whom the background check resulted in a response that would cause a student to be denied to student teach.

PLEASE BE AWARE THAT HOPE COLLEGE DOES NOT RECEIVE THE ACTUAL RESULTS OF CRIMINAL BACKGROUND CHECKS FROM PESG.

If you had a criminal background check completed within the last academic year for a paid position (coaching, summer camp, etc.), you will need to submit a Fingerprints Transferred to PESG (Form 12) at the above link to your former employer.

NOTE: OUT OF STATE PLACEMENTS

Students who are participating in the Chicago and Philadelphia Semester Programs MUST be fingerprinted in the respective states as part of the application process for each individual program.
