INSTRUCTIONS FOR FINGERPRINTING AND REGISTERING WITH THE OTTAWA AREA ISD FOR STUDENT AND SUBSTITUTE TEACHING

You will need an agency number along with reason code to be printed. You can access this information by downloading the “Livescan Fingerprint Request Form” from the OAISD website (see web instructions below).

Fingerprinting can be done locally at the locations listed below using the following registration information:

Ottawa Area ISD Agency Code: 5385L
Reason option: SE-School Employment, MCL 380.1230

• Ottawa Area Intermediate School District (OAISD), at the Educational Services Building located at 13565 Port Sheldon Street, Holland through IBT (formerly Identix Identification Services) on MONDAYS BY APPOINTMENT ONLY ($65.25). Phone number: 866-226-2952 or schedule your appointment on-line at www.iisfingerprint.com

• Holland Hospital through IBT (formerly Identix Identification Services) ON FRIDAYS BY APPOINTMENT ONLY ($65.25). Phone number: 866-226-2952 or schedule your appointment on-line at www.iisfingerprint.com

• Ottawa County Sheriff Department at the Fillmore Complex located at 12220 Fillmore Street, West Olive (between Waverly/120th and US 31). Phone number: 738-4000. WALK IN between 8:00-4:30 Monday-Friday: NO APPOINTMENT NEEDED

(Scroll down to Fingerprinting Process - select “click here” for the “Livescan Fingerprint Request Form” that includes Agency ID # and Reason Code).
• Student Teaching

You may print off the packet on-line but you must submit the paperwork to the OAISD in person. You must bring the following documents with you:

• a receipt from agency who completed your fingerprinting
• a transcript
• a driver’s license and one other piece of ID (passport, social security card, birth certificate).
