What forms do I need for my off-campus program? What do I do with the forms when they're complete?
  • Check the Required Paperwork page for a complete list of necessary forms and procedures, sorted by type/length of program.
How often do my students need to complete these forms?
  • EVERYTIME you take students off-campus, you need to make sure the appropriate paperwork is completed.
    Exceptions to this rule are:
    - Athletics: one set of forms per student, per season for the regular season only.
    - Class Field Trips: one set of forms per student, per semester. Please submit a new itinerary for each program.
    - Research Programs: one set of forms per student, per research program. Please submit a new itinerary for each program.
